Who needs D4 medical check?

A Taxi medical is a health check required when applying for or renewing your taxi driver licence. It ensures you meet the medical standards to drive safely while carrying passengers.

This applies to drivers of:

  • Hackney Carriages (Taxis)

  • Private Hire Vehicles (PHVs)

How Often Do I Need a Taxi Medical?

This varies by local authority. To find out how often you need to undergo this medical, contact your local authority or check their website.

What Does the Medical Involve?

Medical History Questionnaire

You’ll be asked about your medical history, including any past or current medical conditions. You’ll also be asked about any regular medications you’re taking. If you’ve had any scans, tests, or specialist reports, you may be asked about these — bringing copies of results will be helpful. Most conditions, if well managed, may not affect your application.

Blood Pressure Check

Your blood pressure will be measured to ensure it meets the DVLA Group 2 standard. Your reading must not consistently exceed 179/99. If your systolic pressure (top number) is consistently 180 or higher, or your diastolic pressure (bottom number) is 100 or higher, you will not meet the required standard.

Vision assessment

Your vision will be tested using a Snellen chart to check it meets requirements for driving.
If you wear glasses or contact lenses, please bring them with you, along with your most recent prescription.

Completion of D4 form by the docter

Based on your medical history, medications, and the results of your examination, the doctor will complete your form. Typically, the entire process takes around 20 minutes to complete.

It is crucial that you bring a summary of your medical records to the appointment. Without these records, we cannot proceed with your medical examination. You can obtain a summary of your medical records from your GP’s reception.

Form of Photographic ID e.g Drivers License or Passport.

A summary of your medical records (available from your GP’s reception) – mandatory for your medical examination

Your Glasses or Contact Lenses (if you use any) and their Prescription

The specific form from your Local Authority (can be obtained directly from them or via their website)

A list of any medications you are currently taking
