Who needs a D4 medical?

A D4 medical is a health assessment required by the DVLA for anyone applying for or renewing a commercial vehicle license. It ensures you meet the physical requirements to drive safely. This includes drivers of:

  • Heavy Goods Vehicles (HGVs) or Large Goods Vehicles (LGVs) like Lorries 
  • Passenger Carrying Vehicles (PCVs) like Buses, Coaches, Minibuses
  • Motorhomes
  • Ambulances
  • Some Forklift drivers

How often do I need a D4 Medical?

How often you need a D4 medical depends on your age:

  • Under 45: You need a D4 medical when applying for your first licence. After that, you usually won’t need another unless you have a medical condition or the DVLA has requested one.
  • 45 to 65: You’ll need a D4 medical every 5 years to renew your licence.
  • Over 65: Once you’re 65, you’ll need a D4 medical every year to renew your licence

You may also need a D4 medical if you have a medical condition that affects your ability to drive safely, or if the DVLA asks for one.

What does a D4 Medical involve?

Medical History Questionnaire

You’ll be asked about your medical history, including any past or current medical conditions. You’ll also be asked about any regular medications you’re taking. If you’ve had any scans, tests, or specialist reports, you may be asked about these — bringing copies of results will be helpful. Most conditions, if well managed, may not affect your application.

Blood Pressure Check

Your blood pressure will be measured to ensure it meets the DVLA Group 2 standard. Your reading must not consistently exceed 179/99. If your systolic pressure (top number) is consistently 180 or higher, or your diastolic pressure (bottom number) is 100 or higher, you will not meet the required standard.

Vision assessment

Your vision will be tested using a Snellen chart to check it meets Group 2 requirements for driving.
If you wear glasses or contact lenses, please bring them with you, along with your most recent prescription.

Completion of D4 form by the docter

Based on your medical history, medications, and the results of your examination, the doctor will complete your D4 form. Typically, the entire process takes around 20 minutes to complete.

Form of Photographic ID e.g. Drivers License or Passport.

Your Glasses or Contact lenses (if you use any) and their Prescription

A list of any medications you are currently taking

Medical records if you have any ongoing conditions, including the most recent letter from any specialists you see regularly, and any scan or test results if available.
